Course Content
• The wider social determinants of health and their impact on the physical, mental, and emotional wellbeing of individuals, families, and communities
• The causes of mental, emotional, and physical ill-health, long-term conditions, disability and premature death in the local community, their risk factors, and the opportunities for prevention and management
• The nature and boundaries of the role when representing the interests of people using the service.
• How to manage relationships in a referral pathway, the expectations of the referrer, and the person being referred.
• Relevant legislation and how it influences policy and protocols when promoting or protecting the community health.
Course Assessment
During your apprenticeship you will be trained internally by your employer, assessed, and observed in the workplace by Dudley College assessors and complete scenarios, discussions, and a marked written portfolio of work. You will also need to pass an assessment at the end of the programme and successfully complete a observation of practice and professional discussion about the evidence that you have collected throughout your apprenticeship. This end point assessment will be carried out by an independent company but will be arranged by Dudley College

You will achieve a Pearson level 3 Standard in Community Health and well-being.

Progression
This standard provides the foundation into number of career paths into health and science sector including public health Practitioner, lead/management roles and you can progress onto level 5 Coaching Professional standard.
Entry Requirements
To undertake this apprenticeship, you will be required to have a DBS, and be employed in a suitable care setting.
Apprentices will also need to acquire Level 2 functional skills in English and Maths unless they already have grades A-C/4-9.
