Community Health and Wellbeing Workers work in partnership with individuals, families and communities to identify health and wellbeing needs, prevent ill-health and connect people to the services and support they need. Community Health and Wellbeing Workers help individuals and communities take greater control of their health and wellbeing by building confidence, identifying strengths, improving access to services and supporting positive behaviour change. They work across health, social care, local authority and voluntary sector organisations.
What will Apprentices Learn?
Apprentices develop the knowledge, skills and behaviours required to support individuals and communities to improve their health and wellbeing.
Training combines workplace experience, professional coaching, project work, portfolio development and structured off-the-job learning. Apprentices apply their learning directly within their role, creating immediate benefits for employers and communities.
The programme is delivered in the workplace.

- Health inequalities and community wellbeing
- Physical, mental and emotional health promotion
- Behaviour change approaches
- Asset-based and strengths-based working
- Community engagement and development
- Partnership working and referral pathways
- Signposting and service navigation
- Public health and prevention strategies
- Safeguarding and professional practice
- Data, information sharing and record keeping
- Legislation, policy and community health frameworks
Apprentices learn how to work alongside individuals and communities to identify unmet needs, build resilience, improve wellbeing and access appropriate support services.
Learning combines expert teaching with practical workplace application, helping apprentices make a positive impact from day one.
We'll work closely with you to ensure the programme supports both organisational priorities and community outcomes.
Qualifications included
As part of the programme apprentices will achieve:
- Pearson Level 3 Community Health and Wellbeing Worker Apprenticeship Standard
- Level 2 Functional Skills in English (where required)
- Level 2 Functional Skills in Maths (where required)
These qualifications support the development of competent practitioners who can work confidently across health, care and community settings.
Entry Requirements
Apprentices should be employed in a suitable role that allows them to demonstrate the knowledge, skills and behaviours required by the occupational standard.
Apprentices should also hold a grade 4 or equivalent in both GCSE English and maths (or Functional Skills in English and maths).
Apprentices may be required to work towards English and maths qualifications as part of their apprenticeship where appropriate. We will provide advice on the requirements that apply to your apprentice and support them throughout their learning journey.
How is the Apprentice Assessed?
Apprentices are assessed throughout the programme through workplace activities, observations, discussions and portfolio development.
The End-Point Assessment (EPA) includes:
• Observation or Demonstration of Practice
• Professional Discussion
• Knowledge Assessment
These assessment methods confirm the apprentice has achieved the knowledge, skills and behaviours required by the occupational standard.
